本研究は、結核病棟に勤務する看護師からみた結核患者の療養生活とその心理過程を記述的に明らかにすることを目的とした。協力の得られた大阪府内の4施設の結核病棟に勤務する看護師11名を対象に、結核患者の療養生活に関して半構造化面接を実施し、面接内容を逐語化して質的分析を行った。なお、本研究は、大阪市立大学大学院看護学研究科、および各対象施設の倫理委員会の承認を得た。分析の結果、【入院生活を送ることが困難な患者の傾向】【疾患特有の揺れ動く患者の心理状態】【内服することの大変さ・困難さ】【疾患受容ができない】【行動変容の難しさ】【退院後の治療継続の困難さ】の6つのカテゴリーが抽出された。患者の心理状態を把握した上での指導や支援、医療者との関わり、患者が社会との接点や変化を感じられる入院生活を送ることができるためのより具体的な看護介入を検討していくことが必要とされる。 |

The purpose of this qualitative study was to describe the nurses' perception about hospital life and psychological processes of inpatients with active tuberculosis (TB). Semi-structured interviews were undertaken with 11 nurses who worked at TB ward from 4 hospitals in Osaka. Participants were asked how they feel about TB patients and their hospital life. Interviews were audio-taped, transcribed verbatim and analyzed by qualitative description. As a result, six categories contributing to interpretations of hospital life and psychological processes of TB inpatients were identified from the qualitative analysis of the interview data: the tendency of the patients having difficulty with sending hospitalization life; The patients's insecure feelings; the difficulty of taking medicine; the difficulty with illness acceptance; The difficulty of positive change in health behavior; the difficulty of successful medication/treatment without interruption. These findings suggest that people providing care to TB inpatient need to better understand their psychological condition and support needs. Especially, it is important to maintain social relationships of TB inpatients and the adequacy of the supporting relationship between patients and nurses. |
